General Requirements For Medical Child Support Order Qualification

There are general qualification requirements that a medical child support order must meet. In addition, the order must satisfy the employer's Qualified Medical Child Support Order (QMCSO) requirements. Otherwise, the order will be denied. Get the employer's QMCSO requirements on this site or contact us.

For a medical child support order to be qualified, it must contain the following information:

  • The employee's name, last known mailing address, and Social Security Number
  • The name(s) and last known mailing address of the child(ren) covered in the order or the name and address of the child(ren)'s representative
  • The child(ren)'s full Social Security Number or last 4 digits of the Social Security Number
  • A description of the type of coverage to be provided to the child(ren)

In addition, the order must:

  • Be signed or stamped with the judge's signature
  • Contain a seal and/or stamp of the court clerk indicating that the document is a copy of the original on file with the court
