Terminating a QMCSO
A New QMCSO is Required
To terminate a child(ren)'s coverage before the terms of the Qualified Medical Child Support Order (QMCSO) are completed, a party must submit a new QMCSO stating the previous one is terminated. The new QMCSO must be issued from the same court that sent the original QMCSO and must contain the following information:
- The employee's name and Social Security Number
- The full name(s) of the child(ren) whose QMCSO coverage is to be terminated and/or the case number that appears on the original QMCSO
- A statement that the originally submitted QMCSO is to be terminated

What Happens If a QMCSO Is Terminated
- Coverage under the QMCSO will end for the eligible child(ren), according to a date determined by the employer.
- The Qualified Order Center will notify all people named in the QMCSO and their designated representatives that the original QMCSO will be terminated.
- If the employee voluntarily covered the child(ren) under the health plan(s) before the QMCSO, the eligible child(ren) will continue to be covered.
What Happens If a QMCSO Isn't Terminated
- The employee will remain obligated by the terms of the existing QMCSO.
- The Qualified Order Center will notify all people named in the QMCSO and their designated representatives that the termination of the QMCSO isn't effective and enclose specific denial reasons.
- A revised court-certified QMCSO may be submitted to:

- A determination of the revised QMCSO will be made 15 business days after it's received.
